Direct Cremation Package for $695.00  DEATHS OCCURING @ HOSPITALS or COUNTY MORGUE
INCLUDES: Transportation from any hospital or any county morgue in Washtenaw County during normal business hours.  Transport to the crematory.   Filing the death certificate with the Washtenaw County Clerk.  Notifying Social Security Administration with a SS-721 Statement of Death.  Cardboard cremation casket that the decedent is placed in for cremation.  The crematory fee along with the Washtenaw County Medical Examiner permit fee and a basic plastic urn to hold the cremains.

INCLUDES: Immediate attention & transport from any nursing home or care facility in Washtenaw County at any time of day or night.  Transport to the crematory.   Filing the death certificate with the Washtenaw County Clerk.  Notifying Social Security Administration with a SS-721 Statement of Death.  Cardboard cremation casket that the decedent is placed in for cremation.  The crematory fee along with the Washtenaw County Medical Examiner permit fee and a basic plastic urn to hold the cremains.  PLEASE NOTE THAT CREMATION SERVICES REQUIRING IMMEDIATE TRANSPORT FROM THE LOCATION OF DEATH MUST BE PREARRANGED & PREPAID.

INCLUDES: Immediate attention & transport by a two person transfer team from a private residence in Ann Arbor, MI, at any time day or night.  Transport to the crematory.   Filing the death certificate with the Washtenaw County Clerk.  Notifying Social Security Administration with a SS-721 Statement of Death.  Cardboard cremation casket that the decedent is placed in for cremation.  The crematory fee along with the Washtenaw County Medical Examiner permit fee and a basic plastic urn to hold the cremains.  PLEASE NOTE THAT CREMATION SERVICES REQUIRING IMMEDIATE TRANSPORT FROM THE LOCATION OF DEATH MUST BE PREARRANGED & PREPAID.
